Support At Home
Support At Home assists you to live safely and comfortably in your own home. We start with a friendly conversation, create a personalised care plan, and match you with a caring team who fits your routine. Your support can change as your needs change.
From 1 November 2025, the Australian Government will introduce a new aged care funding model known as the Support at Home Program. This program will replace the current Home Care Package system.
Looking ahead, and no earlier than 2027, the Support at Home Program will also gradually replace the Commonwealth Home Support Program (CHSP), creating a single, streamlined system for in-home aged care support.
At A1 Community Care Providers, we’re here to make this transition as smooth and stress-free as possible. Our team is committed to helping you understand the changes, navigate your options, and continue receiving the support you need to live safely and confidently at home.
✔ Eight levels of in-home support, with funding of up to $78,000 per year, plus two short- term support pathways
✔ Quarterly budgets, replacing the current annual funding structure
✔ A clear and transparent service list, so you know exactly what services are available to you
✔ Price caps on services, expected to begin around July 2026 (providers may set their own prices until then)
✔ A 10% cap on care management fees, helping ensure more of your funding goes directly toward your care.
SAH Funding
Under SAH, an older person’s funding level is determined following an aged care assessment. The assessment considers the person’s daily living needs, health and clinical requirements, mobility and safety risks, and support goals.
The outcome is an assigned funding level (1–8). Higher levels are intended to support more frequent services, increased complexity, and greater reliance on care at home.

The table below summarises each SAH level. Funding amounts shown are indicative ranges only (subject to government updates). Actual budgets and permitted service categories should be confirmed at the time of onboarding.

Your Contributions Explained
Under the Support at Home (SAH) aged care program, client contributions are based on your individual financial circumstances, as assessed by Services Australia.
Your contribution level helps determine how much you may pay toward the cost of your care and services.

Care management now capped
Under the Support at Home (SAH) program, care management fees are capped at 10% ensuring the majority of your funding goes exactly where it should, directly into the quality care and supports that helps you live well at home.
Care management covers essential coordination activities such:

✔ Coordinating services and supports
✔ Ongoing monitoring of care needs and clinical support
✔ Liaison with families, carers, and health professionals
The capped amount means there is a limit on how much of your budget can be used for care management, providing transparency, fairness, and value for money.
We are committed to keeping care management costs clearly explained, with no hidden fees. Our focus is on maximising you’re funding so it supports your wellbeing, independence, and goals.
Unspent Funds Explained
As part of the new Support at Home program, you’ll receive a quarterly care budget to use on the services that best support your needs, goals, and independence.
Each quarter, you can carry over a portion of any unspent funds up to 10% of your quarterly budget or $1,000 (whichever is higher). This gives you added flexibility to plan ahead and ensures your care continues smoothly without disruption.
If you are transitioning from an existing Home Care Package, any unspent funds will move with you, so nothing is lost and your care can continue seamlessly.

Additional Services available under SAH
Under the Support at Home program, additional services may be available to support more specific or changing needs. These can include restorative care to help maintain or regain independence, end-of-life care focused on comfort, dignity, and family support, and Assistive Technology and Home Modifications (AT-HM) to improve safety, mobility, and day-to-day living at home.
Access to these services is based on assessment and eligibility, and we will guide you through the process to ensure the right supports are in place when needed.
Restorative Care
A short-term program designed to gently support you in rebuilding strength, confidence, and independence after illness, injury, or time in hospital.
End-of-Life Care
Compassionate, respectful care provided in your home, with a focus on comfort, dignity, and supporting what matters most to you and your loved ones.
