Our Story

At A1 Community Care Providers, we are more than a service. We are people who have parents, grandparents, and loved ones of our own. When we support someone, we always ask ourselves one simple question:

How would we want our own family to be cared for?

That question shapes every decision we make.

We believe care should feel personal. We look beyond forms, funding, and checklists to truly see the individual. Every person has a story, a routine, a home, and a way they like things done. Our role is to respect that, protect it, and support it.

We are committed to keeping people comfortable in the place they know best — their home — for as long as possible. Familiar surroundings bring confidence, stability, and peace of mind, and we work gently alongside each person to maintain their independence while ensuring they feel safe and supported.

Our team is carefully chosen not only for skill, but for character. Trust matters in care, which is why we prioritise consistency and matching participants with support workers who genuinely suit them.

The values that guide us every day:

01.

Quality & Compliance:
We hold ourselves to high professional standards so families can feel confident their care is safe, reliable, and accountable.

02.

Dignity & Respect:
Every interaction is delivered with patience, kindness, and understanding. We honour personal choice and individual routines.
03.
100% Transparency in Communication:
We speak openly and honestly. Families are kept informed, questions are welcomed, and nothing is hidden behind complicated language.
04.
Keeping People at Home Longer
Our focus is on supporting independence and wellbeing so people can remain in familiar surroundings for as long as possible.

05.

Trusted Support Workers

We carefully select and support our staff so the people entering your home are dependable, compassionate, and respectful.

At A1 Community Care Providers, care isn’t just about assistance — it’s about reassurance, relationships, and making each day feel a little easier.

Support At Home

What is Support At Home

Support At Home assists you to live safely and comfortably in your own home. We start with a friendly conversation, create a personalised care plan, and match you with a caring team who fits your routine. Your support can change as your needs change.

From 1 November 2025, the Australian Government will introduce a new aged care funding model known as the Support at Home Program. This program will replace the current Home Care Package system.

Looking ahead, and no earlier than 2027, the Support at Home Program will also gradually replace the Commonwealth Home Support Program (CHSP), creating a single, streamlined system for in-home aged care support.

At A1 Community Care Providers, we’re here to make this transition as smooth and stress-free as possible. Our team is committed to helping you understand the changes, navigate your options, and continue receiving the support you need to live safely and confidently at home.

What the Support At Home Program includes:

✔ Eight levels of in-home support, with funding of up to $78,000 per year, plus two short- term support pathways

✔ Quarterly budgets, replacing the current annual funding structure

✔ A clear and transparent service list, so you know exactly what services are available to you

✔ Price caps on services, expected to begin around July 2026 (providers may set their own prices until then)

✔ A 10% cap on care management fees, helping ensure more of your funding goes directly toward your care.

SAH Funding

How Support At Home funding level applied

Under SAH, an older person’s funding level is determined following an aged care assessment. The assessment considers the person’s daily living needs, health and clinical requirements, mobility and safety risks, and support goals.

The outcome is an assigned funding level (1–8). Higher levels are intended to support more frequent services, increased complexity, and greater reliance on care at home.

The New Funding Levels (1–8)

The table below summarises each SAH level. Funding amounts shown are indicative ranges only (subject to government updates). Actual budgets and permitted service categories should be confirmed at the time of onboarding.

Care management now capped

Under the Support at Home (SAH) program, care management fees are capped at 10% ensuring the majority of your funding goes exactly where it should, directly into the quality care and supports that helps you live well at home.

Care management covers essential coordination activities such:

✔ Care planning and regular reviews

✔ Coordinating services and supports

✔ Ongoing monitoring of care needs and clinical support

✔ Liaison with families, carers, and health professionals

The capped amount means there is a limit on how much of your budget can be used for care management, providing transparency, fairness, and value for money.

We are committed to keeping care management costs clearly explained, with no hidden fees. Our focus is on maximising you’re funding so it supports your wellbeing, independence, and goals.

Unspent Funds Explained

As part of the new Support at Home program, you’ll receive a quarterly care budget to use on the services that best support your needs, goals, and independence.

Each quarter, you can carry over a portion of any unspent funds up to 10% of your quarterly budget or $1,000 (whichever is higher). This gives you added flexibility to plan ahead and ensures your care continues smoothly without disruption.

If you are transitioning from an existing Home Care Package, any unspent funds will move with you, so nothing is lost and your care can continue seamlessly.
